The undersigned hereby applies for the following category of annual membership at Urban Golf:

During normal operating hours, all Urban Golf Memberships include:
q Unlimited use of the indoor driving range
q Unlimited use of the indoor putting and chipping green
q 10% Discount on simulator hourly rates
q 10% Discount on instruction hourly rates
q Book tee times on simulators up to 2 weeks in advance
q 24 Hour cancellation policy for simulator tee times
q Book private events up to 1 month in advance
q Enrollment in the Urban Golf loyalty points program
NOTE: Areas of the driving range or putting green may be reserved for instruction or special events by Urban Golf from time to time without notice.
Membership Payment
Memberships are sold on an annual basis and automatically renew unless otherwise cancelled by the member. There are no refunds upon cancellation of any memberships. Annual renewal periods will always start on the first day of the month in the month one year after the month your membership was purchased. Membership fees will not be prorated based upon the day of the month the membership is purchased. Example: Single membership purchased on May 10, 2007 or May 30th, 2007 will renew on June 1st, 2008.
An annual renewal notice will be sent via email the 1st day of the month prior to the annual automatic renewal month. Members will have ten (10) calendar days to provide email notice to Urban Golf to cancel the automatic renewal. If Urban Golf does not receive email cancellation notice or other written cancellation notice from member within the ten (10) day period, then the member will be automatically renewed for another annual membership.
Applicant agrees to maintain a current credit card account in Applicant´s name on file with Urban Golf at all times. Urban Golf will automatically charge Applicant´s credit card account on the 1st day of their annual automatic renewal month. Notwithstanding anything herein to the contrary, Urban Golf may immediately revoke Member´s membership for a default in the annual payment of membership.
Assumptions of Liability
It is understood and agreed that Applicant is assuming no liabilities whatsoever in connection with this membership other than the payment of the sums set out above, any applicable renewal membership dues and charges incurred by Applicant, Applicant´s family and Applicant´s guests in the use of Urban Golf and that such membership does not confer upon Applicant any ownership, interest, or rights of any nature in the Urban Golf property or assets. Applicant understands that any use of the facilities at Urban Golf by Applicant, Applicant´s family and Applicant´s guests is done at their risk, and Urban Golf is not liable for personal injury, theft, or loss of personal property, as further provided in the Membership Plan.
Urban Golf Rules and Regulations
Applicant hereby acknowledges that Applicant has received, has read, and understands the Membership Agreement and the Urban Golf Rules and Regulations (collectively the "Membership Documents") applicable to this membership, which are incorporated below for reference.
1. Members, players, and guest play and practice at their own risk.
2. Children under the age of 16 must be accompanied by parent or guardian.
3. All simulator tee times require credit card for reservation.
4. All members and players are responsible for 100% of any simulator hours booked and not cancelled in advance. Members have 12 hours and players have 24 hours in advance to cancel without charge.
5. A separate fee is required for non-members to use the indoor driving range and indoor putting and chipping green. Please inquire at the front desk for rates.
6. Appropriate clothing must be worn at all times in designated golf areas. We do not allow tank tops, cut off jeans or other inappropriate apparel. Athletic footwear which covers the entire foot must be worn at all times.
7. Loud, profane, or abusive language is not permitted. Urban Golf reserves the right to refuse service to anyone.
8. Smoking and smokeless tobacco products are not permitted inside Urban Golf. The outside patio may be used.
9. Please limit use of the driving range and putting greens to one hour if others are waiting.
10. All emergencies, injuries, or broken equipment must be reported to the manager on duty.
11. Urban Golf equipment and golf balls may not be removed from the club.
12. Golf instruction may be given by contracted instructors of the club only.
13. Urban Golf reserves the right to discontinue any programs or activities due to concerns with safety or other conflicts with the operation of the club.
Applicant hereby agrees that Applicant´s use of the Urban Golf facilities and privileges under the membership are subject to the terms, conditions and restrictions set forth therein. Applicant agrees to conform to and abide by said Membership Documents as same may be amended from time to time. Urban Golf reserves the right, in its sole and absolute discretion, to terminate memberships in the Club, to discontinue operation of any or all of the Urban Golf facilities, to sell or otherwise dispose of the Urban Golf facilities in any manner, and to make any other changes to the terms and conditions of the membership agreement or use of the Club facilities.
Membership Signature
I understand and hereby agree to these terms of membership at Urban Golf. My membership will terminate on the expiration date of the last period for which I have paid. I further agree to abide by all the Urban Golf rules and regulations and that failure to abide by these rules and regulations are grounds for cancellation of my membership. No refunds will be allowed (no exceptions). Your signature binds this membership.
I authorize Urban Golf to charge my credit card the full amount of the balance due.
